    From an Acute Care Hospital

If a patient in an acute care hospital needs rehabilitation, or long term acute care, the referral process is as follows:

    • A physician, case manager, or other clinician from the acute care hospital requests that the patient be evaluated by a HealthSouth liaison.
    • A HealthSouth liaison conducts a comprehensive screening to determine if the patient meets criteria for acute admission. The screening includes a chart review, discussion with healthcare personnel, and a meeting with the patient and/or family to address pertinent information, concerns, and questions.

    Admissions from Home

Referrals from home offer healthcare providers additional treatment options. Because inpatient rehabilitation and long term acute care is considered acute hospitalization , patients typically qualify for admission, even if they are currently at home. Inpatient treatment is more intensive and focuses on activities of daily living and recovery, while receiving 24-hour specialized nursing care, so patients can return home with greater function and independence.
